Community HealthChoices is Pennsylvania’s new managed care program for anyone who currently receives BOTH Medicare and Medicaid.
If you receive Medicare and Medicaid, enrollment in Community HealthChoices is mandatory, and must be completed by November 13, 2017. The plan will then take effect on January 1, 2018 in south- western Pennsylvania.
HOWEVER, if you are currently enrolled in a LIFE Program (such as LIFE Armstrong County, LIFE Beaver County, LIFE Butler County or LIFE Lawrence County), you are NOT required to enroll in Community HealthChoices, as LIFE Programs are an approved option for the plan.
Current LIFE Program participants are required to TAKE NO ACTION during the Community HealthChoices enrollment period. All of your LIFE Program services will continue without changes or disruptions.
If you are not enrolled in a LIFE Program, now is the time to consider this affordable, all-inclusive health plan that allows eligible* seniors to remain safely in their homes. Your complex medical, functional and social needs are managed for you.
Services available to you can include:
- Comprehensive medical care
- Adult day services
- In-home support care
- Prescription coverage
- Transportation to the LIFE Center where your medical needs can be met, meals are served and you can participate in a variety of recreational activities.
